Introduction

Lady Reading Hospital Peshawar is the largest hospital in the Province. It has been taken as a pilot project for computerization. The desired system involves online information availability on network within the hospital and a portion of the information for public view on the internet. The objective of the project is to help the Hospital staff in improving patient related information management and processing for better services.

The re-engineering and automation requirements in the following areas, namely:-

Fully computerization of the Lady Reading Hospital to ensure the reliability, security, services and to improve the patient treatment system and its record.

Up-grade/purchase computing/networking hardware, licensed software Internet access for medical research purposes.

Connect all the units of hospital in a computerized network and to keep record of all the data.

Provide research facilities to doctors like clinical audit.

Facilitate managers in decision-making to maintain online decision support system.

Establish communication between management and clinical staff.

Ensure availability and provision of online doctor’s appointments, duty schedules and OPD information to public.

Arrange online clinical opinion for patients, doctor’s discussion forums on various clinical issues.

Focus mainly on computerization of patients/clinical information system and to make all patients records online to identified users.

Following modules have been automated:-

The objective is to automate the patient registration, outpatient management system, inpatient management system. This project will facilitate the General public in all aspects of health. A complete record of patient is maintained at central data base which can be retrieved any time. Doctors can use the patient data for different kinds of researches. This project will also help in policy making for health in future on the basis of computerized data.

The system includes the following functional modules:

Out Patient Module

This module consists of Patient Registration form, Patient Visit form, Treatment form, Diagnoses and Prescription forms. All patients at Registration Counter OPD and Parchi Counter Casualty are registered using Patient Registration form, while patients are given a unique ID on the basis of which the patient record can be maintained and updated for admission and different clinical tests.

Daily revenue report can also be generated using the patient registration form at OPD Registration Counter and Parchi Counter.

Inpatient Module

This module entertains all the patients who have been admitted into the different units/wards of the hospital using the same Unique ID provided to the patient at Registration Counter or Parchi Counter.

This module consist of the following forms

Patient Admission ward and bed number is allotted to the patient using this form.

Initial Assessment case summary, clinical diagnoses of the patient are recorded.

Daily Progress Detail daily general information of the patient “i.e. temperature, blood    pressure, medicines etc” is recorded in to the software using this form.

Patient Transfer this form is used to transfer a patient from one unit to another.

Refer to Operation this form is used to transfer a patient from ward to OT.

Operation Operation notes of the patient are recoded using this form.

Clinical Services Module

This form is consist of 3 sub modules

Pathology all the inpatient and out patient test requests are entertained here along with    revenue generation reports.

Radiology all the inpatient and out patient X-Ray and Ultra sound requests are    entertained here along with revenue generation reports.

Blood Bank all the in patient blood request is entertained using this sub module.
